Our copay grants offer funds to cover your out-of-pocket medication costs. Through our copay grants, we offer a fast, reliable way to get help paying for medications.
How copay grants work
We provide grants for a 12-month period, in full compliance with all relevant laws, regulations, and government guidelines. If you still need more funds to cover your medication costs, you can apply for additional funding based on availability.
When you receive a PAN copay grant, you can choose your covered medications based on the treatment plan prescribed by your provider, as long as the medication is covered by your disease fund. You can also choose the providers you use and which pharmacies you use.
The amount of copay assistance you can receive varies by disease fund.

What’s covered
Our copay grants cover your out-of-pocket costs of the medications listed in the disease fund’s formulary that you apply for. We cover prescription medications for each diagnosis if they meet these criteria:
- FDA-approved
- Listed in official compendia
- Listed in published evidence-based or clinical guidelines
The grants do not cover medications or over-the-counter products that are not covered by your insurance, medications not listed in the formulary for your disease fund, medications already paid 100% by your health insurance, medications billed only to drug discount cards, and medical services and office visits.

Eligibility requirements vary by disease fund. Select a specific disease fund to find full eligibility requirements.
To be eligible for a copay grant, you (or your patient) must meet these criteria:
- You’re getting treatment for the disease named in the disease fund or program.
- You have the specified health insurance that covers your qualifying medication or product. Note: health insurance requirements vary by disease fund.
- Your prescribed medication must be listed as a covered medication within the disease fund you apply for.
- Your income must fall at or below the federal poverty level specified by the disease fund.
- You must reside and receive treatment in the United States or U.S. territories. Note: U.S. citizenship is not required.

How to apply
Step 1
Find your disease fund and check your eligibility with our easy pre-screening tool.
Step 2
If you have previously applied for a PAN grant or joined one of our disease fund wait lists, you can apply online through the PAN portal or by phone. If not, please apply by phone.
Healthcare providers and pharmacies can apply online through the PAN portal or by phone on behalf of their patients.
To apply by phone, call us at 1-866-316-7263 Monday through Friday, 9 a.m. to 5:30 p.m. ET.
Be prepared with this information for your application:
- Your diagnosis and medications
- Your health insurance carrier and insurance member ID
- Your personal and contact information, including date of birth, mailing address, email address, and social security number
- Your adjusted gross annual household income
- Your provider’s contact information, including name and phone number

How to renew your grant or apply for additional funding
You must be an active grant recipient to apply for additional funding or a renewal grant for an open disease fund.
Wait lists
If your fund is closed and you are eligible for that fund, sign up for the wait list. When the fund opens, we’ll contact you right away.
Before signing up for the wait list, check the fund’s eligibility criteria to make sure you qualify. The wait list does not guarantee eligibility, so checking your eligibility in advance can avoid delays when the disease fund opens.

Other PAN grants
Transportation grants
If you already have a copay or premium grant, you can apply for a transportation grant. The transportation grant helps PAN patients pay for transportation to and from activities that improve their overall health. This can include transportation to and from grocery stores, pharmacies, medical appointments, social support, and more.
Health insurance premium grants
PAN offers health insurance premium grants to pay for insurance premiums for some disease funds. Learn more about these funds.
Additional funding and renewal grants
You must be an active grant recipient to apply for additional funding or a renewal grant for an open disease fund. Learn about eligibility and how to apply.
Partners and other support
The PAN Foundation partners with other organizations, called alliance partners, who provide education and support for specific disease states.
We also provide information about how you can take advantage of the Social Security Administration’s Extra Help program, also known as the Low-Income Subsidy. The Extra Help program helps people on Medicare drug plans pay for the monthly premiums, annual deductibles, and copays related to their prescription medications. We encourage you to explore our partners and other support resources, in addition to PAN’s assistance programs.
